Empowering Families and Individuals
At WJS Canada, our Family Resource Centres (FRCs) offer inclusive and accessible support to families and individuals, helping them navigate disability services, access critical resources, and work toward greater independence.
Our Approach
We follow a structured and updated process to ensure every person seeking support receives the right support at the right time:
1. Referral
We accept referrals from Disability Services and self-referrals. For self-referrals, we work with families to confirm eligibility for PDD or FSCD services and guide them through the intake process to identify their needs and available supports.
2. Intake and Assessment
During intake, we assess the individual’s needs to determine the services and training to help them achieve their goals. This step includes explaining rights, responsibilities, and the scope of services.
3. Personalized Service Delivery
Based on the needs assessment, we provide customized support and training to meet the individual’s goals. Regular check-ins help ensure progress, and services can be adjusted based on feedback.
4. Ongoing Review and Support
Throughout service delivery, we review the individual’s progress and make necessary adjustments to ensure their needs are being met.
5. Completion and Closure
Services are concluded when individuals have met their goals, request to withdraw, or stop engaging with the service. We ensure that this process respects the individual’s preferences.
6. Self-Referral Process
Families can now connect directly with FRCs through a streamlined self-referral process. Here’s how it works:
- Eligibility Check: We confirm that the individual or child is eligible for PDD or FSCD services and has an open file.
- Service Fit: We determine whether the requested supports fit within the services offered by the FRC.
7. Verification:
If both criteria are met, we complete the FRC Self-Referral Verification Form to initiate support.
